The Mayo Clinic EmbodyHealth Campaign 2007 will run through January 21, 2008. If you have taken the HRA in 2007, you will be able to compare your data and progress. The HRA can be taken once during each year of the Campaign.

This will take you to the Mayo Clinic EmbodyHealth web portal.
If you need to go directly to the portal the address is: www.NASAHealthierYou.com. - As a first time user go to New Visitor, otherwise go directly to Returning Visitor.
- You will need a valid individual e-mail address and password. Employees may use their nasa.gov email address. The e-mail address is used to sign up for and receive supportive e-mails from Mayo Clinic.
- During registration you will be asked for a unique ID. Please contact Gail Bantugan at gail.a.bantugan@nasa.gov or 321-867-3164 to obtain the unique ID.

You will obtain an email confirmation from Mayo Clinic once you agree to the terms of use.
Note:Unfortunately for some users, this e-mail may be flagged as "spam" by the NASA e-mail filters and may automatically be directed to a "Junk" e-mail folder. If after a short period of time (about 30 minutes), you don't see the confirmation e-mail from Mayo in your regular mail box, please check your "Junk" mail folder. - Completing the HRA should take about 15 minutes! After completion, you will be provided with a Personal Health Report and guidance.
- You may complete the HRA any time between February 18, 2008 and January 31, 2009. The HRA may be taken once a year.
- Important! In order for the Mayo Clinic EmbodyHealth website to work correctly you must disable the pop-up blocker in your browser. Please remember to enable pop-up blocking after you finish using the Mayo Clinic EmbodyHealth website.
Privacy and Confidentiality of Information
The information you enter and the use of the resources is private and confidential. NASA will not have access to your personal health data. The only data available to NASA from the Mayo Clinic will be aggregated such as group level risk prevalence.
